GRADE A FOOD EXCELLENCE AWARD |

This is an award for food service operations and retail food establishments. This distinction is to publicly acknowledge the highest level of commitment by any food operation in the area of food safety and sanitation. As you will know after reviewing the requirements, this award not only requires a track record of good sanitation, but, emphasizes education of an operation’s food handlers.
Operations earning this distinction will be recognized in two ways. First, they will receive an attractive certificate (8.5” by 11”), which can be framed and hung up for customers to view. Secondly, a listing of all award winners will be listed on our web page and yearly newsletter.

Consideration for this award is initiated by the operation applying. The environmental director will ultimately review all applications. Operations will be notified in writing what criteria were lacking should the award be delayed. The health department wants to recognize all operations that successfully meet the criteria and are committed to food safety and education.
This award will have to be renewed yearly by the Health Department administrative staff. The operator will have to maintain the criteria to have the award reissued on a yearly basis. Notification of renewal will be sent to the operator.
Any operation within the jurisdiction of Stark County Health Department may request consideration for the Grade A Food Safety Award on an application provided by the health department. Call Paul DePasquale for an application at 330-493-9904 ext. 224 or the Health Department website if your operation can meet the following criteria:
1. One person, working a minimum of 30 hours per week at the facility, must be ServSafe (or equivalent per Ohio Department of Health) certified.
2. A written procedure that outlines a food safety education and training program must be submitted.
3. No more than 2 critical violations for the past calendar year for establishments <25,000 sq. ft. and no more than 3 critical violations for establishments >25,000 sq. ft.
4. No more than 4 non-critical violations for a level 1, 2 or 3 food operations and no more than 8 non-critical violations for a level 4 operation.
5. Health Department administrative staff will determine criteria compliance.
